I think this is about depression, and feeling generally disconnected from everyone but also enraged and frustrated that there is nothing that helps. There are also references to substance abuse, and how depression has made him addicted to attention, drinking, meds, whatever he thinks could "keep him warm" in the winter of the soul.
Toward the end there is an allusion to death (possibly suicide), but he wants to resist the urge.
Also, it's obviously a take on "Shine on, You Crazy Diamond" by Pink Floyd (and it has a "Comfortably Numb" reference, which is cool).
